Please write it down or print out so you don't forget it. ※完了画面でブックマークをしても、表示が変わるので確認することはできません。 Even if you bookmark the completion screen, the display will change and you will not be able to check it. 予防接種をご予約される方 Those who are making a reservation for vaccination 接種できるワクチンのみが表示される便利なワクチン管理予約です。 This is a convenient vaccine management reservation that displays only available vaccines. 接種間隔やお子さんの年齢から自動的に接種可能なワクチンのみ表示されます。 Only vaccines that can be administered automatically will be displayed based on the vaccination interval and the child's age. 今までに接種した履歴の入力が必要になりますので、母子手帳を見ながら正確にご入力ください。 You will need to enter your vaccination history, so please check your maternal and child health handbook and enter the information accurately. 履歴のみを入力されるときは、予約メニューの【予防接種履歴の入力】からお進みください。 If you would like to enter only your vaccination history,please proceed from Enter vaccination history in the reservation menu. ・なるべく事前に予診票を記載のうえ、お越しください。 Please fill out the pre-examination from before coming. ・必ず定期接種予診票と母子手帳をご持参ください。 Please bring your regular vaccination pre-examination card and maternal and child health handbook. ・予診票と母子手帳をお忘れの場合は接種できませんのでご注意ください。 Please note that if you forget your pre-examination ticket and maternal and child health handbook, you will not be able to receive the vaccination.
